a) If used on a compilation, what other artists will appear on the album? Identify the artists for which permission has already been obtained.
b) If a film, video, DVD, or commercial, what exactly is it about? Where will the music you want to license appear in the project? If other music will be included, from which other artists?
c) If background music, describe the precise nature of the subject? If other music will be included, from which other artists?

3) Which Real Music title/s would you like to use (which song/s from which album/s)?

a) How much of the song/s will be used, i.e. 20 seconds, entire song/s, song repeated, etc. (indicate total time)?

4) If the material will be sold or broadcast:
a) To whom will it be sold/broadcast and how will it be distributed/broadcast?
b) In what countries will it be sold/broadcast?

5) If a product, how many units will be manufactured? If to be sold, retail price?

6) If to be shown or broadcast, how many times will it be shown, in how many locations/stations?

7) What is the total budget for the project? Music budget?

8) What is the term (length of time) you are seeking to have a license for the music?

9) What is the name and description of your organization or company?

10) What year was this organization or business founded? What is the web site address for this organization?
